About this file

This document is a Safety Data Sheet template for a federal contract opportunity. The form requires offerors to provide comprehensive safety information, including OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) Rates and Total Recordable Case (TCR) Rates for the previous five calendar years. Offerors must detail their Safety Management System approach, including management leadership, employee involvement, hazard prevention and control, worksite analysis, and safety training. The form also requires descriptions of subcontractor selection processes and safety compliance oversight.

The associated federal contract opportunity is a Navy solicitation (N0025325Q0009) for replacing two restroom doors and frames at the Naval Undersea Warfare Center Division in Keyport, WA. The solicitation is a small business set-aside with an estimated contract value between $15,000 and $50,000, targeting a June 10, 2025 award date. It is classified under NAICS code 238350 (Finish Carpentry Contractors) and will use a Low Price Technically Acceptable evaluation method. Offerors must be registered in the System for Award Management (SAM) and can submit inquiries to specified Navy contract representatives.
