About this file

This is a Stocking Survey Data Form template used by the USDA Forest Service to collect and evaluate reforestation monitoring data on the Malheur National Forest. The form captures contractor-collected field data on planted and naturally regenerated trees across multiple survey plots, measuring metrics such as species composition, tree viability, tree height, and ground cover conditions (rock, grass, vegetation/shrub percentages). Data collection includes tracking of individual tree counts by species code (Ponderosa Pine, Douglas-fir, Western Larch, Grand Fir, Western White Pine, Lodgepole Pine, and others), classification of trees as viable or non-viable, and calculation of Trees Per Acre (TPA) for both planted and natural regeneration components.

The form establishes a quality assurance and scoring methodology wherein Forest Service Inspector-collected data serves as the foundation baseline, with contractor performance evaluated against this standard on a per-plot basis and then averaged across units. Scoring is calculated as a percentage, with a maximum score of 100 percent possible; any contractor data points falling outside accepted variance thresholds relative to Forest Service measurements result in proportional point reductions. The template includes sections for unit-level narrative descriptions of non-stocked areas, soil conditions, weather damage, stocking level explanations, animal damage impacts, and photographic documentation requirements. The form supports the broader BPA Call Order (1240BG26Q0088) issued against the Land Management Integrated Resources (LMIR) BPA for natural regeneration and reforestation stocking survey services.

Scoring will be done on a per plot basis and then averaged across each unit. The toal point value of FS-Collected Data is the foundation - a score >100% is not possible. The final contractor score for this plot was 28 --> (28/36)*100 =78%

If the Contractor-collected data does not lie within the accepted variance of the Forest Service Inspector's data, the corresponding point value will be reduced accordingly.

In this example, the cells highlighted in yellow delineate metrics the contractor did not accurately measure compared with the Forest Service Inspector's data.
