About this file

This is a Past Performance Questionnaire template used by the U.S. Department of Agriculture (USDA), Rural Development (RD) to evaluate contractor performance for the Multi-family Physical Inspection Program under Solicitation 12RADZ26R0001. The questionnaire is classified as source selection sensitive and collects information on contracts completed within the past three years or currently in performance. Evaluators must assess contractor performance using a five-tier rating scale: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory, with detailed definitions provided for each level. Completed questionnaires must be submitted via email to Gary.brown3@usda.gov within seven days of receipt, with a deadline of 3:00 PM EST on August 22, 2026.

The questionnaire evaluates eight key performance areas: overall project performance, responsiveness and flexibility, experience with government standards and regulations, quality control and inspection procedures, timeliness of deficiency correction, cooperation with government personnel, and ability to meet volume orders in a timely manner. Evaluators must provide comments for all ratings at or below Satisfactory level and may include additional documentation such as past performance database reports or information regarding adverse performance and corrective actions. The form requests respondent information including evaluator name, office location, phone, position/grade, length of time on the program, and completion date. This template serves as a source selection tool for determining best value to the Government in awarding the base plus four option period firm fixed-price services contract.
