About this file

This is a Past Performance Questionnaire used by the U.S. Department of Agriculture (USDA), Rural Development (RD) to evaluate contractor performance for the Multi-family Physical Inspection Program competitive service contract opportunity (Solicitation 12RADZ26R0001). The questionnaire is designed to collect past performance information from references regarding contractors' work on similar contracts completed within the past three years or currently ongoing. Evaluators are instructed to assess contractor performance using five rating levels: Exceptional, Very Good, Satisfactory, Marginal, and Unsatisfactory, with detailed definitions provided for each level. The document requires handwritten responses and mandates submission by August 22, 2026 at 3:00 PM EST to Gary.brown3@usda.gov.

The questionnaire covers eight primary performance evaluation areas: overall project performance, responsiveness and flexibility, experience with government stringent standards and regulations, quality control and inspection procedures, timely correction of deficiencies, cooperation with government personnel, and timely fulfillment of volume orders. Evaluators must provide comments for any ratings below "Very Good" and are encouraged to supplement assessments with organizational knowledge. Section 10 collects respondent information including evaluator name, office location, phone, position, program tenure, and completion date. The questionnaire also requests attachment of supporting past performance database reports or materials documenting adverse performance and corrective actions. A concluding statement asks evaluators to indicate whether they would recommend future contract consideration with the contractor.
