1.0 GENERAL INFORMATION

1.1 EMERGENCY JUSTIFICATION

During recent monsoonal storm events at the Chino Valley Forest Office and Administration Site, located at 735 North US Highway 89 in Chino Valley, Arizona, there was damage sustained to three of the buildings 8810516A-Chino Yard Office, 8810516B-Supervisor’s Office and 8810564A-Chino Shop.

Damages to the Chino Yard Office include the destruction of the roof, major water damage to interior walls and the ceilings. This building was tested for Asbestos and was found to have 5% chrysotile asbestos in the roofing materials and will need to be abated prior to the roof being replaced. This building provides permanent office spaces for four (4) full-time employees, which include the Forest Safety Manager, the Timber Staff Officer, the acting Assistant Fire Staff Officer, and the Forest Fire and Fuels Planner, who have all been displaced. The long-term displacement of these employees will disrupt and impede the effectiveness of strategic planning for the reduction of forest fuels and could lead to extreme fire conditions that could impact the lives of the public that live adjacent to our forest. These employees are also supporting local logging and biomass markets that support the rural prosperity of the local community.

The Chino Shop sustained damage to the metal roof, and some minor water intrusion beneath the door and around the two garage doors. This building provides office space to four (4) full-time and ten (10) season fire and fuels employees, as well as providing storage for all their firefighting equipment, who provide forest fuels reduction in the forest around this community and who support wildfire suppression during local fire events. Although these employees have not been displaced, if damage is not mitigated immediately, the building will become unoccupiable which could lead to delays responding to wildfires in the community.

The Supervisor’s Office sustained water intrusion from the upper windows and some minor damage to the metal roof. While investigating the roof damage it was found that the upper roof did not have a z-bar flashing. This building provides office space to 12 full-time employees, which include the Forest Supervisor, Deputy Forest Supervisor, the Forest Leadership Team, the Public Affairs Officer, the Fire Staff Officer and others. Currently these employees are not displaced; however, if these issues are not resolved quickly, the water intrusion could worsen, causing mold growth and impacting the health of the employees.

1.2 WORK SUMMARY

This project consists of providing all materials, equipment, labor, transportation, and incidentals for completion of the repair storm damage sustained on three buildings at this site.

The repairs are to include roof replacement, dry wall and insulation replacement, a door replacement, ceiling and insulation replacement, installation of gutters and downspouts, and to replace a portion of the side of the Chino Yard Office; to perform maintenance to the metal roofs on the Supervisor’s office and the Chino Shop; to repair window frames on upper windows, to caulk ALL of windows, and to install a Z bar at the roof line on the Supervisor’s Office; and to gutters and downspouts on the Chino Shop.

This project could also include, if additional funds are available, optional bid items. The optional bid items are exterior painting on the Yard Office; replace the weather stripping on two garage doors and a door sweep on a manway; and if mold is found in the Supervisor’s Office to replace the dry wall and insulation.

1.3 PROJECT LOCATION

The Supervisor’s Office is located at 735 N Highway 89, Chino Valley, AZ 86323, LAT: 34.756125°, LONG: - 112.453483°, and is accessible on paved roads. From the intersection of Highway 89A and Highway 89 in Prescott, AZ, travel north on Highway 89 for approximately 8.75 miles, the project site will be located on the east side of the road. The other two buildings are both located less than 150 feet from the Supervisor’s Office. The Yard office is located behind a locked gate and an access code will be provided at the time of the award of this project.

010003 – MEASUREMENT AND PAYMENT

PART 1 - GENERAL

1.1 – Requirements Measurement and payment for contract work will be made only for and under those pay items included in the SCHEDULE OF ITEMS. All other work and materials will be considered incidental to and included in the payment of the pay items in the SCHEDULE OF ITEMS.

1.2 – Method of Measure One of the following methods of measurement for determining final payment is DESIGNATED on the SCHEDULE OF ITEMS for each pay item.

A. Actual Quantities (AQ): The quantity to be paid is the quantity listed in the bid schedule. The contract quantity will be adjusted for authorized changes that affect the quantity or for errors made in computing this quantity. If there is evidence that a quantity specified as a contract quantity is incorrect, submit calculations, drawings, or other evidence indicating why the quantity is in error and request in writing that the quantity be adjusted.

Final payment will be made for the actual quantities at the unit price shown in the SCHEDULE OF ITEMS, subject to the Variation in Quantities Provision.

B. Lump Sum Quantities (LSQ): These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. They will not be measured.

C. Estimated Quantity (EQ): If the quantity of a unit-priced item in this contract is an estimated quantity and the actual quantity of the unit-priced item varies more than 15 percent above or below the estimated quantity, an equitable adjustment in the contract price shall be made upon demand of either party. The equitable adjustment shall be based upon any increase or decrease in costs due to the variation above 115 percent or below 85 percent of the estimated quantity. If the quantity variation is such as to cause an increase in the time necessary for completion, the Contractor may request, in writing, an extension of time, to be received by the Contracting Officer within 10 days from the beginning of the delay, or within such further period as may be granted by the Contracting Officer before the date of final settlement of the contract. Upon receipt of a written request for an extension, the Contracting Officer shall ascertain the facts and make an adjustment to extend the completion date as, in the judgement of the Contracting Officer, is justified.

1.3 – Pay Units:

One of the following methods of measurement for determining final payment is DESIGNATED on the SCHEDULE OF ITEMS for each pay item.

A. Lump Sum (LS): This pay unit denotes complete unit of work as required by or described in the contract, including all necessary materials, equipment, and labor to complete the job. They will not be measured.

B. Linear Feet (LF):Is a standard unit of measurement representing exactly 12 inches in a straight line.

C. Centum Square Feet (CSF): Is a standard unit of measurement equal to 100 square feet.

D. Each (EA): These quantities denote one complete unit of work as required by or described in the contract, including all necessary materials, equipment, and labor to complete the job. They will not be measured.

Payment will be by units defined and determined according to U.S. Standard measure.
