About this file

This is a Taxpayer Identification Number (TIN) form required by the State of Illinois for contractor registration and compliance purposes. The form serves as a certification document for entities seeking to conduct business with Illinois state and local government agencies, establishing the contractor's correct tax identification information and confirming their status regarding backup withholding requirements from the Internal Revenue Service.

The form accommodates multiple entity types including individuals, sole proprietors, partnerships, corporations, limited liability companies, governmental entities, tax-exempt organizations, and specialized entities such as pharmacies and legal services corporations. Contractors must provide their legal name, business name if applicable, and either a Social Security Number for individuals and sole proprietors or an Employer Identification Number for business entities. For corporations, applicants must attach IRS acceptance letters (Form CP261 or CP277). The form requires the authorized representative's signature and date of completion, and includes certification language confirming the accuracy of the taxpayer identification number provided and the contractor's status regarding IRS backup withholding obligations.
