About this file
This is a Bidder Declaration form issued by the California Department of General Services, Procurement Division, used for state and local contract opportunities. The form requires prime bidders to declare their California certifications (Microbusiness, Small Business, Nonprofit Veteran Service Agency, or Disabled Veteran Business Enterprise), identify whether subcontractors will be used, and describe the distinct elements of work or goods the prime bidder will perform or provide. For the contract opportunity EM-25-113 Weather Tower & Shelter Trailer, bidders must procure one shelter and tower trailer and one 33-foot aluminum weather tower. If subcontractors are proposed, bidders must complete a detailed table listing each subcontractor's name, contact information, address, California certification status, description of work or goods provided, percentage of bid price, good standing verification, and rental equipment ownership status.
The Bidder Declaration form establishes compliance requirements for certified businesses to maintain commercially useful functions and prohibits brokers or agents from claiming certification benefits unless disabled veteran owners maintain at least 51-percent ownership of materials, supplies, services, and equipment provided. All subcontractors must be verified for valid licenses, permits, California business qualification, and current certification status. The form requires certification under penalty of perjury by an authorized representative of the bidding firm, with signature, printed name, and date. This form is mandatory for all prime bidders responding to California state procurement solicitations and serves as a mechanism to ensure compliance with small business utilization goals and veteran business enterprise requirements.
