Bid Form Summary

This is a Bid Form for a construction contract issued by the Iowa Department of Administrative Services (DAS) for roof replacement work at the Fort Dodge Correctional Facility. The project encompasses partial tear-off and roof replacement on Building K (approximately 7,700 square feet) and the Floyd Unit (approximately 18,300 square feet), involving removal of existing ballast and membrane, replacement of 25% of water-damaged roof insulation and substrate board, mechanical fastening of new polyisocyanurate insulation, installation of new EPDM membrane, and replacement of fascias and gutters with new aluminum trim. The base bid package (BP 01) requires bidders to provide lump sum pricing for all labor, materials, and equipment. Three alternates are available: ALT 01A and ALT 01B for 100% roof insulation replacement on each building, ALT 02 for a deductive price if substantial completion is extended to June 18, 2024, and ALT 03A and 03B for polyisocyanurate cover board additions. Additionally, two unit price items are solicited for water-damaged wood blocking replacement and substrate/insulation replacement at the square foot rate. Bids were due on May 9, 2023, at 2:00 PM, with an optional pre-bid meeting held on April 11, 2023. The anticipated construction period runs from May 17, 2023, to November 30, 2023.

All bid amounts must be stated in both words and figures, with discrepancies resolved in favor of the written amount. Bidders must provide 5% bid security with their submission and certify their status as either resident or nonresident bidders under Iowa Code 73A.21. Bids remain valid for 30 calendar days following the bid due date. All bidders must be registered with the Iowa Labor Commissioner and comply with federal and state affirmative action and equal employment opportunity requirements. The State of Iowa is exempt from state and local sales taxes; therefore, construction material taxes shall not be included in bid amounts. Bidders must identify all subcontractors and suppliers within 48 hours of the bid submission date and cannot change them without owner approval. The solicitation reserves the right for the State to reject any or all bids, accept any combination of alternates, and determine the low bidder based on the sum of the base bid and accepted alternates. Bidders must also provide proof of insurance coverage and required surety bonds upon contract acceptance and comply with all Iowa State Building Code requirements.
