Statement of Work
I. Project Identification
A. Title: Removal of Discontinued USGS Concrete Gage Structure
B. Location: Backyard of residential home located at 208 Depue Street, Belvidere, NJ 07823
II. Objectives
To procure services to dismantle a discontinued data collection station which is a 5’x5’x8’ concrete gage house with a concrete topper. Below the 5’x5’x8’ gage house is a 3’x4’ poured concrete “stilling well” that is approximately 28’ deep, mostly all below grade. Overall total height of structure is 36 ft. The below land portion will need to be filled in with an appropriate material to prevent entry. The awarded contractor will be removing the structure and hauling away all debris including remaining attachments to the gage structure (metal pipe, wooden door and shelf, metal trap door and vent screens).
III. Special Requirements
Access to the site is made by entering the property on Depue Street.
Returning any alterations to the yard or landscaping to “before project” condition will need to be included in this proposal
Filling of well below surface with appropriate material is included in this proposal.
Work under this contract shall be performed in accordance with the requirements of this Statement of Work, specifications for this contract, and all applicable federal, state and local regulations. It is very important that the successful bidder understand that this project includes removal of debris made by deconstruction.
IV. Inspections, certifications and compliance
A. Contractor will be responsible for ensuring that all debris will be removed from the site.
B. Contractor will be responsible for returning area to as close to natural as possible

VI. DELIVERIES OR PERFORMANCE
Commencement, Prosecution, and Completion of Work: Normal working hours shall be 7:00am-5:00pm, Monday –Friday. The contractor shall not work Saturdays, Sundays, legal holidays, or non-normal working hours, unless otherwise authorized by the Contracting Officer’s Technical Representative (COTR). Request to work times other than normal hours shall be made in writing. The contractor shall employ sufficient personnel and equipment and order supplies to accomplish the work of this contract within 90 calendar days of the Notice to Proceed.
Progress Schedule: The Contractor shall submit a project schedule to the Contracting Officer’s Representative (COR) for approval within five days after receipt of the Notice to Proceed. The progress schedule shall be a chart, which graphically depicts the proposed sequence to accomplish each work feature or operation. It shall show start dates and completion dates for each event on a horizontal time scale. The progress schedule time scale shall begin with the Notice to Proceed and indicate the number of days to completion. The schedule will be the reference to which the contractor progress and effort are compared.
SPECIAL CONTRACT REQUIREMENTS
List of Subcontractors: For purposes of monitoring compliance with the insurance and payroll reporting requirements of this contract, the contractor shall furnish a list of all subcontractors to the Contracting Officer.
Transporting of Debris and Material: All materials and construction debris shall be removed from the work area and removed from the site daily.
Disposal of debris: All materials and debris shall be disposed of properly by the contractor.
Organization at the site: The site is located inside the Wells Fargo building and access is controlled by the buildings security and building engineer. The location is on the roof of a building and as such all materials should be stored in accordance with the building engineer.
Site Cleanliness: The contractor shall maintain the site free of construction debris in contractor furnished containers (barrels, bags, dumpsters). The contractor maintain cleanliness of the work site by promptly removing debris, tools, and excess material.
Contractor Parking:
Environmental Protection: The contractor shall perform all work and activities in accordance with local, state, and federal environmental regulations.
Outages and Interruptions: N/A
Notice to the Government of Delays: In the event the contractor encounters difficulty in meeting performance requirements, or when it anticipates difficulty in complying with the contract delivery schedule or date, or whenever the contractor has knowledge that any actual or potential situation is delaying or threatens to delay the timely performance of this contract, the contractor shall immediately notify the Contracting Officer, in writing, giving pertinent details. This data shall be informational only in character. Notice under this provision shall not be construed as a waiver by the Government of any delivery schedule or date or any rights or remedies provided by law or under this contract.
Pre-Construction Meeting: The Contractor shall have the project manager available on site for a pre-construction meeting prior to the commencement of work for this project. The COTR will conduct a pre-construction meeting to review the requirements and conditions of this contract.

All work listed shall be coordinated with the Contracting Officers Representative.
