About this file
This is a Past Performance Questionnaire template designed to evaluate consultant or contractor performance on completed projects for source selection purposes.
The questionnaire is divided into two main sections. Section 1 captures contract identification information, including the consultant name, project type (standalone or part of overall contract), period of performance, initial and final project costs with explanations for cost variances, scope of work description, and evaluator information (name, title, contact details, and months worked on contract). Section 2 evaluates the quality of consultant performance using a five-point rating scale (Outstanding, Good, Acceptable, Marginal, Unacceptable, Neutral/Not Observed). Performance is assessed across two primary categories: Technical Performance (ability to accomplish technical requirements, meet schedule, and provide quality data/reports/analyses) and Management Approach (project/program management effectiveness, personnel cooperation, problem resolution timeliness, workforce qualification and retention, subcontractor management, and quality control). Evaluators place ratings in designated blocks for each performance characteristic, with the option to provide up to two additional pages of supplemental information. The completed questionnaire is designated as source selection sensitive and is restricted to evaluation team use only.
