About this file
This is a Past Performance Questionnaire form for RFQ #140P5126Q0007 that requires offerors to document their relevant experience in providing Landscaping and Groundskeeping Services.
The form mandates that offerors list no more than five completed projects from the past three years, numbered consecutively beginning with the most recent project. For each project, offerors must provide the project name, location, owner name and contact information (including area code and reachable telephone number), dollar value, dates of service, and a detailed description of the project scope and the offeror's work performed. Additionally, offerors must provide a comprehensive narrative explanation of their company's prior experience in delivering Landscaping and Groundskeeping Services comparable to the solicitation requirements. The form emphasizes that offerors must candidly disclose instances where past performance may be considered less than fully satisfactory, accompanied by explanations of remedial corrective actions taken or planned to address any deficiencies. Failure to provide such candid disclosure may result in an unfavorable past performance assessment. The form includes space for three projects with complete information fields on page two, with the header indicating this is a two-page document.
