About this file

This document is a Past Performance Questionnaire form for RFQ #140P5126Q0011 that requires offerors to demonstrate their experience providing Janitorial Services similar to the solicitation requirements.

Offerors must provide a detailed explanation of their company's prior experience in janitorial services and submit information on three completed projects from the past three years, listed in order from most recent to earliest. For each project, offerors must include the contractor/offeror name, project name, project location, owner name and address, reachable telephone number with area code, project value, dates of service, and a comprehensive description of the project scope and the offeror's work performed. The form emphasizes the importance of candid disclosure, particularly regarding any instances where past performance may be considered less than fully satisfactory. Offerors are required to explain any deficiencies and describe the remedial or corrective actions taken or planned to address them. Failure to disclose performance issues or provide complete information may result in an unfavorable assessment of the offeror's past performance record. The form accommodates up to five projects maximum, though only three completed jobs are explicitly required for submission on this template.

Past Performance Questionnaire READ CAREFULLY: The purpose of this form is for the offeror to list services performed during the past three (3) years that are similar to the work described in this solicitation. The number of projects listed shall not exceed five (5) and shall be numbered consecutively, beginning with the most recent project. Use this form to address your familiarity with the Statement of Work. Include Area Codes with all telephone numbers. Fill in the information by typing or printing legibly.

It is especially important that the offeror disclose instances in which their past performance may be considered by others to be less than fully satisfactory. The offeror should tell their side of the story and describe remedial corrective action that has been taken or will be taken to correct the deficiency. Failure to do so may result in a determination that an offeror has been less than candid with the Government, which could result in an unfavorable assessment of the offeror’s past performance record.
