About this file
The document is a Past Performance Questionnaire (PPQ) form used by the Army Contracting Command - Rock Island (ACC-RI) to evaluate a contractor's performance history for a potential contract opportunity. The form is designed to gather detailed information about a contractor's previous contract performance, including quality of work, adherence to schedules, problem-solving abilities, and compliance with security and safety requirements.
The PPQ requires respondents to provide comprehensive details about a specific contract, such as contract type, dollar value, period of performance, and contact information. Ratings are provided across multiple performance categories using a five-tier scale ranging from Excellent to Unsatisfactory. The completed questionnaire must be submitted electronically to Contract Specialists Jason Koski and Laura Spencer, with the subject line clearly indicating the offeror's name and solicitation number W519TC-25-R-0019. The form is part of the evaluation process for a contract related to BLU-136/B 2,000lb High Fragmenting Bomb Assembly and CNU-417/E Shipping and Storage Container.
