About this file
This is a Project Manual for a construction bid package associated with the Indian Health Service (IHS) Crow Hospital Water and Mold Damage Repair project located in Crow Agency, Montana. The contract number is 75H70125F30005, with Bodwe Project No. 241532-B, issued for bid on July 29, 2026. The project is a single prime contract based on stipulated price and involves demolition and renovation of departments and rooms affected by mold and water damage, including Central Sterile Processing, Surgery, Supply and housekeeping rooms, and toilet/locker rooms. Specific work includes demolition and replacement of existing walls, doors, frames, hardware, flooring, cabinetry, and ceiling tiles/grids. Plumbing, electrical, and fire suppression work will be minimal except for complete reconfiguration of the Central Sterile Processing area. HVAC work requires replacement of the existing system while maintaining operations until changeover. The contractor must remove, salvage, and store medical equipment prior to work commencement and furnish and install specified medical equipment per the equipment schedule. The Owner (IHS) intends to continue occupying adjacent building portions during construction and occupy the project upon substantial completion.
The manual includes comprehensive safety and infection control requirements applicable to healthcare facility construction. The contractor must submit an Accident Prevention Plan (APP) addressing hazard identification, competent person designations, training documentation, and activity hazard analyses (AHAs) at least 15 calendar days before the preconstruction conference. A full-time Site Safety and Health Officer (SSHO) with professional safety certification and three years of construction safety experience is required on-site for each shift. Mandatory requirements include pre-placement tuberculosis screening for all contract employees, infection control risk assessments (ICRA) with Class II or higher requiring permits, HEPA-equipped dust control with negative air pressure maintenance for Class III and IV risk areas, weekly safety inspections with formal documented reports, mishap investigation and reporting procedures, and compliance with OSHA standards including fall protection (4-foot threshold), electrical work (de-energized/electrically safe work conditions), lockout/tagout procedures, and confined space entry protocols. The project manual references standard construction specification divisions covering structural steel, carpentry, roofing, fire suppression, plumbing, HVAC, electrical, and other trades with detailed technical and administrative requirements.
