About this file

This is a Vendor Eligibility Check form issued by the State of Florida Department of Transportation prior to contract award for the O.C. Phillips Road Over Brushy Creek Bridge Replacement project managed by Escambia County. The document serves as a pre-award verification checklist to confirm that the selected vendor and all known subconsultants meet eligibility requirements before contract execution. The project seeks Construction Engineering Inspection (CEI) services for a bridge replacement initiative with Financial Project ID 430468-1-68-01 and federal funding designation D319 049 B. The solicitation was released on July 7, 2025, with a question submission deadline of July 25, 2025, and proposal response deadline of August 8, 2025, at 11:30 am. The project is anticipated to commence on August 25, 2025, with an expected duration of 530 days, completing on February 6, 2027. All meetings are conducted virtually via Microsoft Teams, with optional on-site walk-throughs available.

The vendor eligibility verification requires confirmation that the contractor and subconsultants are not listed on multiple state and federal exclusion lists, including the Convicted Vendor List, Discriminatory Vendor List, Suspended Vendor List, Antitrust Violator Vendor List, Scrutinized List of Prohibited Companies, and the Federal Excluded Parties List System in the System for Award Management (SAM). All proposers and sub-consultants must be pre-qualified with the Florida Department of Transportation in Roadway Construction Engineering (Major Work Category 10.1) and Minor Bridge Construction Engineering Inspection (Major Work Category 10.4). The document references compliance with 2 CFR 1200 and 2 CFR 180 regarding federal debarment and suspension standards, as well as Florida Statutes sections 287.135 and 215.4725 regarding boycotts of Israel and activities in Sudan and Iran. The accompanying professional services agreement estimates total contract value at $10,835, with required tasks of $8,850 and optional tasks of $1,985.
