About this file
This file is a Monthly Activity Report attachment for a Recovery Center contract serving individuals with serious mental illness and/or substance use disorders in the South Central Behavioral Health Clinic region of North Dakota. The document provides a standardized checklist and monitoring instrument designed to assess vendor compliance with contractual obligations through monthly meetings between the vendor and the Recovery Center Director. Section 1 outlines mandatory compliance requirements including maintenance of eligible member populations, operation of 40 hours weekly as a drop-in facility, submission of policy and procedure changes, utilization of the State's Recovery Center Handbook, staffing requirements (filled Director and Certified Peer Support Specialist Coordinator positions), employee documentation, state vehicle policy adherence, staff training on rehabilitation philosophy and recovery-oriented care models, and maintenance of a tobacco-free environment. Failure to meet these requirements results in escalating consequences from warning letters to contract termination.
Section 2 details required activities that directly impact monthly payment, including submission of activity calendars at least 10 days before month-end, weekly peer support groups and recovery education groups with documentation, daily structured social activities, monthly member meetings, provision of computer and internet access, social media marketing updates, and timely statistical reporting. The report also references quarterly compliance reports due by the 15th of subsequent quarters, annual compliance reports due by August 1st, and annual consumer satisfaction surveys conducted in May with results submitted by June 30th. The Recovery Center must operate using the SAMHSA-endorsed Consumer-Operated Services model and collaborate with the local Behavioral Health Clinic, which conducts routine facility reviews and provides improvement recommendations. Failure to complete required activities for three consecutive months triggers warning letters and potential disciplinary action including contract termination.
