About this file
This is an Annual Compliance Report template for a Recovery Center serving individuals with serious mental illness and/or substance use disorders in the South Central Behavioral Health Clinic region of North Dakota. The report must be submitted annually by August 1st to the local Behavioral Health Clinic (BHC) Liaison and the Statewide Peer Support Recovery (PSR) Director. The document outlines mandatory compliance requirements and required activities that vendors must maintain to remain in good standing with the contract agreement. Compliance items include ensuring all members are 18 years or older, maintaining minimum 40 hours of weekly operation as a drop-in center, employing a full-time Recovery Center Director and a part-time Recovery Coordinator certified as a Peer Support Specialist, maintaining a tobacco-free environment, adhering to state vehicle use policies, and completing required staff background checks and training. Required activities include developing monthly event calendars, offering at least one formal peer support group and one recovery education group weekly, providing at least one structured social or recreation activity daily, holding documented member meetings monthly, meeting with the BHC Liaison monthly, and providing computers and internet access to staff and members.
Failure to maintain compliance standards results in a tiered enforcement structure beginning with a warning letter requiring immediate correction, followed by disciplinary action up to contract termination if non-compliance is not corrected within 30 days. Required activities directly impact payment, with reduced monthly payments for incomplete activities and potential contract termination for three consecutive months of non-compliance. Additionally, vendors must maintain updated job descriptions, develop and update social media marketing tools monthly, and complete weekly data reporting in a timely manner. The Recovery Center Director and Recovery Coordinator are required to attend conferences during the reporting year, and any changes to organizational charts or policy and procedure manuals must be reported to the state.
