About this file

This document is a Monthly Activity Report template for a Recovery Center operated under an agreement with a state Health Services Consortium (HSC). The report is designed as a comprehensive compliance and activity tracking tool, requiring monthly completion by the Recovery Center Director to document operational adherence to contractual requirements. The template covers two primary sections: mandatory compliance items that could result in warnings or potential contract termination if not met, and required activities that directly impact monthly payment and continued operation.

The report mandates specific operational standards, including maintaining 40 hours of weekly operation, functioning as a drop-in center, utilizing state-approved handbooks, maintaining a tobacco-free environment, and conducting regular peer support and recovery education groups. Additional requirements include submitting weekly statistical reports, using social media marketing tools, holding member meetings, and providing computer access. The document also outlines quarterly and annual reporting obligations, such as Compliance Reports due on specific dates, an annual consumer satisfaction survey in May, and adherence to the SAMHSA-endorsed Consumer-Operated Services model. Non-compliance with these standards can lead to progressive disciplinary actions, ranging from warning letters to potential contract termination.
