About this file
This document is a Quarterly Compliance Report for a Recovery Center contract, involving a state government agency's behavioral health services program. The report is a comprehensive quarterly tracking instrument requiring Recovery Center Directors to document compliance with contractual obligations, with reporting deadlines set for the 15th of the month following each quarter (January 15th, April 15th, July 15th, and October 15th). The report covers critical operational requirements including age restrictions for members (18 and older), center hours (minimum 40 hours per week), staffing configurations, programming activities, and operational standards.
The compliance report outlines specific performance metrics that directly impact contract payment and potential continuation, with provisions for warnings and potential contract termination for repeated non-compliance. Key compliance areas include maintaining a full-time Recovery Center Director, employing a certified Peer Support Specialist Coordinator, conducting weekly structured programming (peer support groups, recovery education, social activities), maintaining documentation, hosting monthly member meetings, providing technological resources, and submitting weekly statistical reports. The document emphasizes strict adherence to state policies regarding staff training, background checks, vehicle use, and maintaining a tobacco-free environment, with each non-compliant response potentially triggering progressive disciplinary actions up to contract termination.
