About this file

This document is an Annual Compliance Report for a Recovery Center, involving a state-level health and human services agency. The report is an annual requirement due by August 1st, to be completed by the Recovery Center Director and submitted to the local HSC Liaison and Statewide PSR Director. The comprehensive compliance document covers multiple operational aspects of the Recovery Center, including membership demographics, operational hours, staffing requirements, policy compliance, programming, and administrative responsibilities for the previous year.

The report outlines specific compliance criteria across two primary sections, with potential consequences for non-adherence including warnings, reduced payments, and potential contract termination. Key compliance areas include maintaining a minimum 40-hour weekly operation, employing a full-time Director and part-time Recovery Coordinator, offering weekly peer support and recovery education groups, maintaining documentation, providing technology access, developing monthly event calendars, conducting member meetings, and completing weekly data reporting. The document serves as a critical accountability mechanism for the Recovery Center, requiring detailed verification of operational standards and performance metrics to maintain contractual compliance and continued funding.
