About this file
The document is a Request for Substitution/Exemption form from the City of Phoenix Public Transit Department. This form is designed to be submitted to the department's Compliance Section for approval when a prime contractor wishes to make changes to Disadvantaged Business Enterprise (DBE) utilization, including releasing, substituting, changing the scope of work, or reducing the amount proposed for a DBE subcontractor. The form requires the prime contractor to provide details about the project, the affected DBE subcontractor(s), and to summarize the request for exemption along with good faith efforts to maintain DBE participation.
The form provides a structured approach for contractors to request modifications to DBE subcontractor arrangements, emphasizing the need for prior approval from the City of Phoenix Public Transit Department. It includes sections for the prime contractor to detail the specific changes sought, such as releasing or substituting a DBE subcontractor, changing the scope of work, or reducing the proposed amount. The document also includes a section for contract compliance officials to review and approve the request, with provisions for adjusting the DBE goal and utilization percentage. The form underscores the city's commitment to maintaining DBE participation and ensuring transparency in the procurement process.
