About this file

This is a Contractor Self-Certification Form issued by the District of Columbia for campaign finance compliance under D.C. Law 22-250, the Campaign Finance Reform Amendment Act of 2018. The form applies to the Construction Services for MPD Compartmented Access Area contract at the Marion S. Barry, Jr. Building. Contractors must certify their political contribution history since November 9, 2022, including any contributions made to the Mayor, Mayor candidates, mayoral political committees, the Attorney General, Attorney General candidates, Councilmembers, and Councilmember candidates, along with disclosure of any District contracts held at the time contributions were made or within one year prior. The form requires contractors to disclose all company principals by name and title, identify any individuals authorized to modify the certification, and attest under oath before a notary public that all information is accurate and complete and that the business entity is not and will not be in violation of the Campaign Finance Reform Amendment Act of 2018.

The self-certification form mandates that contractors maintain current records of all political contributions and provide detailed information for each contribution including the date, amount, and recipient. Contractors must also disclose whether they have been previously determined to be in violation of D.C. Official Code § 1-1163.34a or are currently in violation, and must certify their commitment to ongoing compliance. The form defines "Principal" broadly to include senior officers such as owners, co-owners, presidents, chief executive officers, chief operating officers, chief financial officers, treasurers, members, and partners, while specifically excluding deans of educational institutions and persons serving only as board members. Submission of this completed and notarized certification is a mandatory procurement compliance requirement for bidders on this District construction contract.
