Summary of Attachment L. Reference Checks
This document is a reference check form issued by the State of Maryland for evaluating bidder and offeror responsibility in connection with a one-year Invitation for Bid (IFB) for preventative maintenance and repair services on an R-22 rooftop HVAC system and fuel oil furnace with hydronic and air heating at a commercial facility located on the Eastern Shore in Kent County. Bidders are required to submit one or more references from customers for whom they have provided comparable goods or services within the timeframe specified in the solicitation. Each reference must include detailed information about the customer company, contact person, project specifics, contract value, performance dates, and a comprehensive description of the goods or services provided that are comparable to the requirements in this solicitation. The Procurement Officer reserves the right to request additional references beyond those provided and may determine a bidder to be "Not Responsible" and reject the bid if the references are deemed insufficient. All references must be readily accessible and willing to communicate via email or telephone.
The form provides spaces for a minimum of three references, with the option to add additional reference pages as needed. References used to satisfy any Minimum Qualifications stated in Section 1 of the IFB may also satisfy this reference check requirement. Contact persons identified as references must be knowledgeable regarding the bidder's performance and capable of documenting the bidder's ability to provide services. The information furnished through this attachment and any verification conducted by the Procurement Officer will serve as the basis for determining bidder responsibility and shall be printed clearly for review. No subcontracting is permitted, and bidders are prohibited from sending inexperienced or residential technicians to this legacy commercial system, with a maximum parts markup of 10 percent and no travel time, mileage, or fuel surcharges allowed.
