| Explanation of Fields: |
1. Submitting Organization: Provide the name of the shipbuilder or vendor submitting the question.
2. Point of Contact: Provide the full contact information for the individual(s) that are submitting the questions.
3. Organization Question #: Provide question number. Each question number should be submitted consecutively. All questions shall have a unique number assigned.
4. Indicate whether question is proprietary.
5. Solcitation Reference: Provide the relevant solicitation document(s), section(s) and/or paragraph(s).
6. Question: Provide detailed question.
