About this file

Attachment K is a Foreign Country of Concern Attestation form issued by the Florida Department of Management Services (DMS) for the Mail Services V2 procurement. The form is a mandatory certification required for entities submitting bids, proposals, or entering contracts that would grant access to Personal Identifying Information, specifically attesting that the bidding entity is not owned, controlled, or principally located in a "Foreign Country of Concern" as defined in Rule 60A-1.020, F.A.C.

The attestation form requires an authorized company representative to declare under penalties of perjury that their organization meets specific criteria regarding ownership, business organization, and control, ensuring no substantial connection to designated foreign countries of concern. This document is part of a broader procurement process for a statewide Mail Services State Term Contract, which aims to establish multiple vendor awards across mail processing, courier, and on-site staffing service categories, with strict requirements around vendor eligibility, international business restrictions, and compliance with state procurement regulations.
