About this file

Attachment K is a Foreign Country of Concern Attestation form issued by the Florida Department of Management Services for the Mail Services V2 procurement (PUR 1355). The form requires an officer or representative of an entity submitting a bid, proposal, or contract to certify that the organization is not owned by, organized under the laws of, or controlled by the government of a Foreign Country of Concern. This attestation is specifically required for contracts that would grant the entity access to an individual's Personal Identifying Information.

The form must be signed under penalties of perjury, with the signatory declaring the truthfulness of the statement. It is part of the broader Mail Services V2 solicitation, which aims to establish a statewide State Term Contract for mail-related services. The attestation serves as a critical compliance document ensuring that vendors participating in the state procurement process do not have significant foreign governmental connections that could potentially compromise information security or create conflicts of interest.
