About this file

This is a Past Performance Questionnaire template designed to collect feedback from government evaluators regarding a contractor's performance on previous contracts. The form is part of the solicitation package (030ADV26Q0056) for the Library of Congress Baton System Replacement project and will be submitted to Jacqueline Miller, Contract Specialist, at jacmiller@loc.gov.

The questionnaire captures contract identification information including the contractor name, agency contract number, prime or subcontractor status, agency name, contract type, period of performance, total contract value, and description of services. Evaluators must identify themselves with name, title, email address, and phone number. The evaluation section uses a four-point scale (Excellent, Satisfactory, Poor, N/A) to assess performance across three primary categories: Quality of Product or Service (including requirements compliance, personnel effectiveness, quality control, and subcontractor management); Contract Management and Delivery (covering management effectiveness, timeliness, problem resolution, responsiveness, material management, and deliverable quality); and Transition/Phase-In and Employee Retention/Attraction (addressing resource transition, workforce qualification, and retention capabilities). The form concludes with narrative sections requesting evaluators to identify contractor strengths and weaknesses, provide future hiring recommendations, and share additional comments, followed by signature and date lines.
