About this file
This document is a Past-Performance Questionnaire (PPQ) for a Library of Congress contract opportunity related to Braille Magazine Transcription and Duplication (Solicitation Number: 030ADV25R0039). The questionnaire is designed to gather detailed performance feedback from previous contractors or clients, using a standardized rating system that ranges from Outstanding to Unsatisfactory across ten key performance evaluation areas including contract management, product quality, project effectiveness, timeliness, personnel competence, communication, and cost performance.
The questionnaire requires respondents to provide comprehensive information about their experience with the contractor, including company details, contract specifics, and ratings on multiple performance dimensions. Respondents are asked to complete and return the form via email to Contract Specialist Nikolas Wellstead and Supervisory Contracting Officer James R. Robinson by a specified deadline. The purpose is to assess the contractor's suitability for a firm-fixed-price with economic adjustment (FFP-EA) Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract for creating and distributing braille versions of print magazines within the NLS network.
