About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance for the Library of Congress Researcher Engagement Services contract (Solicitation Number 030ADV26R0045). The questionnaire collects structured feedback from government or company personnel who have worked with the contractor on previous projects to assess suitability for providing researcher assessment, engagement, and analysis services.

The questionnaire uses a five-point rating scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory) plus a Not Applicable option to evaluate ten performance criteria: overall contract management, quality of products and services, project phase management effectiveness, schedule timeliness and compliance, timely report submission, quality of subcontracted efforts, key personnel competence, personnel communication effectiveness, cost performance, and whether major requirements were terminated or rescheduled due to contractor inability to perform. Respondents must provide narrative explanations for any Marginal, Unsatisfactory, or Not Applicable ratings and supplement evaluations with input from others in their organization. Completed questionnaires must be returned via email to Lianne Carroll (lcarroll@loc.gov) with a copy to James Robinson (jarobinson@loc.gov) no later than 12:00 PM Eastern on September 21, 2026. The questionnaire requests general company information including point of contact, contract number, contract type, period of performance, approximate dollar value, and contract description.

Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor did not/has not yet identified corrective actions.
