About this file
This is a State of Hawaii Contractor's Standards of Conduct Declaration form (AG-010 Rev 11/15/2005) that must be completed by contractors doing business with Hawaii state agencies. The document outlines ethical requirements and declarations that contractors must make regarding their relationship with state legislators and employees, specifically focused on potential conflicts of interest and improper representation.
The form requires contractors to declare whether they are legislators or employees, or have controlling interests in businesses with state connections. It prohibits contractors from being represented by former agency employees within two years of their employment, receiving assistance from legislators or employees for fees, or being represented by individuals who were agency employees or legislators within the past 12 months. The document notes that if a contractor "is" a legislator or employee and the contract exceeds $10,000, it must be awarded through competitive sealed bidding (section 103D-302, HRS) or competitive sealed proposal (section 103D-303, HRS). The agency must post notice of intent to award and file with the State Ethics Commission in other cases where conflicts exist.
