About this file

This is a Past Performance Questionnaire template used by the Library of Congress to evaluate contractor suitability for the Creative Exhibit Design Services solicitation (030ADV27R0010). The questionnaire gathers feedback from current or previous customers of prospective contractors to assess their performance on relevant past projects. Responses must be submitted to Olexiy Levchuk at olevchuk@loc.gov with a copy to Moji Adejuwon at moad@loc.gov no later than November 16, 2026, at 12:00 PM Eastern.

The questionnaire requests general information about the contractor and customer organization, including company name, point of contact details, telephone number, contract number, contract type, period of performance, approximate dollar value, and contract description. Respondents rate contractor performance on ten criteria using a five-point scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). These criteria include overall contract management, quality of products and services, project phase management, schedule timeliness and compliance, report submission timeliness, subcontractor quality and effectiveness, key personnel competence and communication, cost performance ability, and whether major contract requirements were terminated or rescheduled due to contractor performance failures. Narrative explanations are required for any Marginal, Unsatisfactory, or Not Applicable ratings. The Library emphasizes that past performance is of considerable importance to this acquisition and requests honest assessments based on knowledge supplemented by organizational judgment.

Good
Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.

2. Please provide narrative explanations for any Marginal, Unsatisfactory or No ratings and supplement your knowledge of the contractor’s performance with judgement of others in your organization.
