About this file

Service Level Agreement for Contact Center as a Service RFP

This Service Level Agreement document establishes performance standards and operational requirements for Kentucky's Contact Center as a Service RFP 758 27000000097. The agreement defines the service delivery expectations, system availability requirements, and performance metrics that the selected vendor must maintain throughout the contract term. Key performance indicators include system uptime commitments, average response times, call abandonment rates, customer satisfaction scores, and quality assurance standards. The SLA specifies measurement methodologies, reporting requirements, and baseline performance targets that the vendor must achieve and maintain during the initial contract period and any subsequent renewal periods.

The agreement establishes remedies and service credits for non-compliance with defined performance thresholds, detailing the consequences when the vendor fails to meet specified service levels during any given measurement period. Performance monitoring mechanisms include regular reporting cadences, data validation procedures, and dispute resolution processes for disagreements regarding performance metrics. The document outlines responsibilities for both the state and the vendor regarding performance tracking, measurement accuracy, and documentation retention. Penalty structures and service credit calculations are clearly defined based on the severity and duration of performance failures, with escalating consequences for repeated violations of the established service level standards.

Attachment I – Service Level Agreement RFP 758 2700000097 – Contact Center as a Service (CCaaS)

Attachment I – Service Level Agreement - Contact Center as a Service Overview This Service Level Agreement (SLA) defines the Key Performance Indicators (KPIs) used to evaluate Vendor performance under the contract resulting from RFP 758 2700000097 for Contact Center as a Service (CCaaS) solution and associated Managed, Augmented, and System Services. All payment and credit mechanisms are governed by Attachment A § 3.5 (Payment Requirements and Service Level Agreements). This SLA establishes the metrics, measurement methods, and reporting expectations supporting those provisions.

Performance Measurement and Reporting Supporting Reports All supporting data shall be maintained in an Agency-approved repository consistent with the reporting cadence defined in Attachment A.

The specific data source and methodology for each KPI will be defined in the approved reporting format.

Application of Service Level Credits (SLCs) Governance The process for notification, cure, remediation, and final SLC assessment is established in Attachment A § 3.5.2(A)(2). This SLA does not restate that process; it identifies the metrics to which SLCs may apply.

If Commonwealth policy or system changes materially affect performance obligations, the Parties shall review and adjust affected KPIs accordingly.

SLC Cap and Calculation Where a single incident impacts multiple KPIs, the Commonwealth will apply only the single highest applicable SLC unless the affected KPIs measure independent service areas.

Temporary KPI threshold adjustments may be granted for pre-approved events such as major migrations or extraordinary call-volume surges, as documented in the Performance Report Card.

Incident Severity Level Definitions

1. Severity 1 – Critical A complete outage or severe system degradation that prevents core business operations.

No workaround is available.

2. Severity 2 – High A major service degradation affecting a substantial portion of users or an essential function.

Operations continue but are significantly disrupted.

A temporary workaround may exist.

3. Severity 3 – Moderate A partial impairment impacting a limited number of users or a non-essential function.

Operations continue with minor disruption.

A workaround is typically available.

4. Severity 4 – Low A cosmetic issue, enhancement request, documentation inquiry, or other non-operational item.

No impact on business operations.

Cure and Recurrence Provision No SLC shall be assessed for violations caused by factors outside the Vendor’s control. The Agency may waive an SLC at its discretion.

In the event of two (2) failures of the same KPI, the Vendor must perform an internal executive review, determine a method to address the issue, and provide an acknowledgement of the review and summary of vendor actions to the Agency Project Manager within five (5) business days, Repeated or systemic failures of the same KPI in three (3) consecutive measurement periods constitute a pattern of non-performance. Upon such recurrence, CHFS may:

5. Retroactively assess the applicable SLC for all occurrences, even if previously waived;

6. Require a Corrective Action Plan (CAP) under Attachment A § 3.5.3; and

7. Impose enhanced SLCs or payment holds in accordance with this SLA’s Escalation provisions.

Key Performance Indicators and Service Level Credits The KPIs are divided into three sections: Design, Development, and Implementation (DDI); System Maintenance and Operations (M&O), and Contact Center Operations (CCO).

Design, Development, and Implementation (DDI) Assessed per milestone following acceptance of all deliverables within a milestone and receipt of an invoice with supporting documentation in accordance with Section 3.6 of RFP Attachment A.
