About this file

This is a Section 504 Rehabilitation Act Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the Fixed and Removable Prosthesis for Dental Patients procurement opportunity. The document requires contractors to certify compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations. Contractors must complete the assurance form by providing their name, street address, city, state, and zip code, along with an authorized official's signature, title, and date. The assurance is binding on the contractor, its successors, transferees, and assignees, and contractors must acknowledge that contracts will be extended in reliance upon the representations and agreements made in the document.

The document distinguishes between contractors employing fewer than 15 persons and those employing 15 or more persons. Contractors with 15 or more employees must designate a Section 504 Compliance Coordinator and provide that person's name on the assurance form. For smaller contractors with fewer than 15 employees, an exception applies allowing referral of disabled persons seeking services to other accessible providers if no method of compliance exists without significant alteration to existing facilities. All contractors must certify that the information provided is complete and correct to the best of their knowledge at the time of signing.
