About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing compliance requirements for contractors working with San Mateo Medical Center on dental prosthesis services. The document serves as a binding agreement requiring all contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The assurance applies to contracts awarded after the document's issuance date and is binding on contractors, their successors, transferees, and assignees.
Contractors must certify their compliance status by completing the form, with different requirements based on employee count. Contractors employing fewer than 15 persons must simply verify their status and sign the assurance. Contractors employing 15 or more persons are required to designate a Section 504 Compliance Coordinator and provide that person's name and contact information. The DHHS regulations include an exception for contractors with fewer than 15 employees who determine that facility accessibility compliance would require significant alterations; such contractors may alternatively refer disabled persons seeking services to other accessible providers. Authorized officials must sign and certify the accuracy of the submitted information.
