About this file
This is a progress report template for contractors working with the Department of Veterans Affairs (VA) Integrated Veteran Care division within the Veterans' Health Administration. The template outlines the required sections and content structure for regular contractor reporting.
The template requires contractors to provide basic contract information (contractor name, contract number, COR details, period of performance) followed by four main sections: Contract Summary (2-paragraph overview of work status), Medical Documentation Compliance Summary (details on medical documentation compliance during reporting period), Risks and Issues Summary (including risk register, scores, probability, impact and responses), and Corrective Actions and Improvements (listing actions taken to address previously identified issues and resulting improvements). The template is designed to track contractor performance, identify risks, and document remediation efforts for VA healthcare-related contracts.
