About this file
This document is a Pre-Lease Fire Protection and Life Safety Evaluation form from the General Services Administration (GSA) for assessing an office building's fire safety compliance. The form is divided into two parts: Part A for buildings below the 6th floor (to be completed by the Offeror) and Part B for buildings on the 6th floor or higher (to be completed by a professional fire protection engineer). The evaluation requires a comprehensive review of the building's fire protection systems, including automatic fire sprinklers, fire alarm systems, exit signs, emergency lighting, elevators, and other critical safety features.
The detailed assessment covers multiple aspects such as building construction type, occupancy classifications, vertical openings, means of egress, automatic fire suppression systems, fire alarm systems, and elevator safety features. The professional engineer must provide a narrative report identifying any citations, violations, building characteristics, potential fire hazards, and compliance issues. Both the Offeror and the fire protection engineer must sign statements attesting to the accuracy of the information and committing to address any identified deficiencies before the Government accepts the offered space. The form is valid for 5 years, with the acceptance being conditional on no major modifications occurring to the building.
