About this file
This document includes an attachment for a past performance questionnaire and details of a federal contract opportunity. The past performance questionnaire is a template for contractors to provide to references to assess and evaluate the contractor's past performance on relevant contracts. It requests general contractor information and performance ratings on a scale of 1 to 5 in areas such as management, quality control, regulations, problem resolution, schedule, and warranty response. The federal contract opportunity is a solicitation for design-build services for site preparation work for an MRI replacement project at a Department of Veterans Affairs medical center. The opportunity is set aside for verified service-disabled veteran-owned small businesses. The period of performance is 270 calendar days with a project magnitude between $500,000 to $1,000,000. Proposals are due by December 3, 2021 and will be evaluated using a two-phase design-build source selection process and a trade-off methodology providing best value to the government.
