About this file

This is an attachment establishing credentialing and privileging requirements for contractor personnel providing psychiatric physician services under an Indian Health Service contract.

The contractor must ensure all proposed psychiatrists meet minimum qualifications including: a Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited institution; a current, unrestricted medical license in a State, Territory, or District of Columbia; a current Drug Enforcement Administration (DEA) registration appropriate for scope of practice; Board Certification or Board Eligibility in Psychiatry; clinical experience as required by the Statement of Work; and no adverse licensure actions, exclusions, suspensions, revocations, or restrictions. No contractor personnel may begin performance until all credentialing, privileging, onboarding, and security requirements are successfully completed and written authorization is provided by the Government.

The contractor must submit a complete credentialing package for each proposed provider that includes government-issued photo identification, medical school diploma, graduate medical education and residency documentation, current professional licenses, board certification documentation, current DEA registration, current curriculum vitae with complete employment chronology and no unexplained gaps, professional references as required by the Medical Staff Office, verification of relevant employment history, professional liability claims history and malpractice carrier information, current certificate of insurance, and authorizations for NPDB, OIG, and SAM exclusion screenings. Privileges are granted through the Indian Health Service credentialing and privileging process, with the Government retaining sole authority to grant, deny, limit, suspend, or revoke privileges. The contractor must maintain all licenses and credentials throughout performance, immediately notify the Contracting Officer and COR of any restrictions or adverse actions, and provide replacement personnel meeting all contract requirements if a provider fails credentialing or privileging or becomes unavailable. The contractor bears all costs for replacement personnel and must complete security requirements including identity verification, background screening, facility access, information security, and privacy training as directed by the Government.
