About this file
This is a Vendor Statement of Agreement form related to a Request for Proposal (RFP) solicitation issued by the Kentucky Department of Corrections for Reentry Service Centers and Recovery Kentucky Centers. The document establishes the formal agreement between vendors and the state agency regarding the terms of engagement for halfway houses and recovery services. Vendors are required to sign and acknowledge their understanding of the fixed pricing structure established in the RFP solicitation.
The agreement explicitly requires vendors to accept only the pay rates specified in the RFP solicitation with no allowance for rate increases during the contract term. Vendors who attempt to negotiate or secure pay rate increases are considered to be in material breach of any resulting contract. The document functions as a binding commitment form that vendors must complete by providing their company name, authorized signature, title, printed name, and date, thereby formalizing their acceptance of the fixed-price terms outlined in the solicitation.
