About this file
The document is a Third Party Privacy Questionnaire for the University of Massachusetts Chan Medical School and ForHealth Consulting, designed to assess potential vendors' privacy and security practices for a contract opportunity. The questionnaire is comprehensive, requiring detailed information about the vendor's approach to handling Protected Health Information (PHI) and Personal Information (PI), including employee background checks, privacy policies, training protocols, breach response procedures, and subcontractor management. The document is part of a 2024 Request for Applications (RFA) to develop a Qualified Consultant List for prequalifying individuals and small consulting firms to support ForHealth Consulting's strategic initiatives.
The privacy questionnaire seeks extensive details about the vendor's organizational practices, including the number of employees, confidentiality agreements, privacy and security policies, background check processes, and potential international data handling. Key areas of focus include cyber-liability insurance, employee and subcontractor training on compliance and privacy, breach notification procedures, and mechanisms for protecting sensitive information. Vendors must attest to their ability to comply with privacy regulations, provide details about their privacy officer, describe their policies for handling external information requests, and confirm that collected personal information will be used exclusively for the contracted purposes. The document serves as a critical screening tool to ensure potential consultants meet rigorous privacy and security standards before being considered for the ForHealth Consulting contract.
