About this file
This document is a Past Performance Questionnaire for the U.S. Department of Homeland Security's Federal Protective Service, related to a solicitation for an Intrusion Detection System (IDS) installation at the Gerald R. Ford Federal Building and U.S. Courthouse in Grand Rapids, Michigan. The questionnaire is designed to collect detailed feedback from previous project references about a contractor's performance, covering aspects such as professionalism, customer service, problem resolution, project management, quality control, and overall reliability.
The form requires references to provide comprehensive comments on the contractor's performance across 8 key evaluation areas, including professional attentiveness, responsiveness, problem-solving capabilities, supervision/staffing, and handling of complex project requirements. The final section allows the reference to rate the contractor's overall performance as either "Highly Acceptable", "Acceptable", or "Unacceptable" based on the vendor's risk level compared to standard competency expectations. The completed questionnaire will be emailed to Jennifer Rayca and Nikki Finney at the Federal Protective Service for review as part of the contractor selection process.
