About this file
This is a Past Performance Questionnaire template designed by the Federal Bureau of Investigation to evaluate contractor performance on previous relevant contracts and projects. The questionnaire is completed collaboratively, with offerors providing initial contract and service information in Sections I and II, which is then verified and supplemented by an assessor (Government or commercial personnel with direct experience on the contract). The assessor completes Sections III through VI in their entirety and submits the completed questionnaire as a PDF to swalker4@fbi.gov and qcgray@fbi.gov using the naming convention "Vol_1_PPQ_Company Name."
The assessment evaluates contractor performance across six key categories: Program Management (overall management effectiveness, management responsiveness, and personnel management); Technical Performance (quality of technical approach and understanding of requirements); Timeliness of Performance; Quality/Customer Satisfaction (quality of performance and business behavior); Communication; and Cost Control. Each category uses a standardized five-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), or Not Applicable (N). The questionnaire requires the assessor to provide their name, title, contact information, role, and years of experience on the contract, followed by detailed performance ratings and general comments that do not exceed the page limit specified in the RFQ instructions.
