About this file

This is a Disclosure of Subgrantees form (Attachment E) issued by a state or local government agency for sourcing event 0000011989 dated August 7, 2026. The form applies to grantees receiving funding for projects or programs and requires mandatory disclosure of all subgrantees who will perform work under the grant agreement. The disclosure requirements encompass any individuals or organizations engaged to provide goods or services directly related to the funded grant, excluding general administrative services such as information technology, accounting, and custodial functions. Grantees must provide the name of each subgrantee, a description of the type of work or services the subgrantee will perform, and a copy of any agreement executed between the grantee and the subgrantee.

Failure to disclose subgrantee contracts or interests may result in disqualification from consideration for the grant. This disclosure requirement applies to the Code Enforcement Outreach Program (CEOP) focused on citywide landlord engagement in San Francisco, California, which seeks proposals from local non-profit organizations with demonstrated experience in code enforcement outreach. The program aims to assist landlords and property managers in understanding and complying with San Francisco Housing Code requirements. Grantees selected under this opportunity must maintain transparency regarding all subgrantee relationships and contractual arrangements to remain eligible for funding.
