MSIA REQUIREMENTS

Version No.1

Please note that following would need to appear on the face of the COI and on a scheduled additional insured endorsement with “Various locations owned / managed by Crown” for the “Locations of Covered

Operations” section of the schedule that relates to the CG2037 endorsement.

Scheduled endorsements and COI to include:

Crown Castle USA Inc., and all of its parent companies, joint ventures, subsidiaries, and affiliates, herein after “Crown” and property owners

Certificate Holder: The insurance “Certificate Holder” shall be addressed to, Crown Castle USA Inc., Legal Department, 2000 Corporate Drive, Canonsburg, Pa 15317

Exclusions: A copy of the Dec Sheets listing the forms and exclusions associated with the GL and Umbrella policies (if not follow form) would need to be provided. Unacceptable Exclusions are typically associated with

Excess / Surplus lines carrier providers (non-admitted policies / providers). Of concern are specific exclusions that limit / remove protections for bodily harm / property damage related to the following: Employers’ Liabili ty, Employees, Independent Contractors, Subcontractors, Cross Liability, Lease workers, to name a few.

In particular, these are a listing of some (but not limited thereto) of the exclusions that are typically problematic:

Cross Liability Exclusion (anything other than Named Insured versus Named Insured would not be acceptable)

Employer’s Liability Exclusions or limitations

Action Over Exclusions

Injury to Worker Exclusions (as Workers Comp. does not respond to claims made against Crown Castle)

Independent Contractor / Subcontractor limitations (if such limitations affect premiums only of the insured and not coverage – it may not be a problem)

Work Height Exclusions

Operations Exclusions / Limitations

Limitations / Exclusions related to Contractual Liability
