About this file

This is an EEV (E-Verify) Certification and Agreement form required by the Mississippi Transportation Commission in compliance with the Mississippi Employment Protection Act, Section 71-11-3 of the Mississippi Code of 1972. The document outlines certification requirements for vendors contracting with the Mississippi Transportation Commission to verify their participation in the federal work authorization program (E-Verify) operated by the U.S. Department of Homeland Security.

The document requires vendors to provide their company name and EEV identification number, maintain compliance records, and verify subcontractor compliance. Violations can result in contract cancellation, ineligibility for state contracts for up to three years, loss of licenses or permits for up to one year, and potential felony prosecution with penalties including imprisonment of 1-5 years and fines ranging from $1,000 to $10,000. The form must be notarized and includes spaces for authorized officer signatures, dates, and notary information.
