About this file
This is a mandatory Technical and Management Proposal template for the ARPA-H Emerging Health Innovators (EHI) Initiative solicitation (ARPA-H-SOL-26-150). The template establishes specific formatting and content requirements for all full proposal submissions, with non-compliance potentially resulting in non-conforming determinations. Proposals must not exceed eight pages (excluding cover sheet, table of contents, bibliography, and specified attachments), be submitted in PDF, ODX, DOC, or DOCX formats, use 12-point font minimum with 1-inch margins on 8.5x11-inch paper, and be written entirely in English. The Government will not review pages exceeding the page limit, though submissions with fewer pages will not be penalized.
The proposal must include four mandatory sections: (1) Proposal Summary (recommended 2 pages) addressing the intended work, current limitations, novelty of approach, technical risks and mitigation strategies, anticipated impact, specific applications, equitable access considerations, and potential misuse concerns; (2) Technical Plan (recommended 5 pages) presenting a credible plan to achieve project goals with discussion of technical challenges and solutions; (3) Capabilities/Management Plan (recommended 1 page) summarizing team expertise, sub-awardees, consultants, identifying a principal investigator, and describing organizational structure with roles and responsibilities; and (4) optional Bibliography containing key personnel qualifications, relevant papers, references, and reports. Proposers must complete the cover sheet with organization type selection (for-profit, non-profit, or academia), SAM.gov UEI, points of contact, total proposed cost, place of performance, submission date, and proposal validity period (minimum 120 days), along with sub-awardee and consultant team member information.

Instructions:
Use of this template is mandatory for all full proposal submissions to this ISO. Failure to use this template or follow the instructions herein may result in the submission being determined non-conforming. Additional information regarding a non-conforming determination may be found in Section 3.6 of the ISO.
This document must include all components described herein and must be submitted in .pdf, .odx, .doc, or .docx formats. All submissions must be written in English and all pages shall be formatted for printing on 8-1/2 by 11-inch paper with 1-inch margins, and font size not smaller than 12 point. Font sizes of 8 or 10 point may be used for figures, tables, and charts. Instructional text (identified in brackets) may be deleted. The Government will not consider pages in excess of the page count limitations, as described herein. Proposals with fewer than the maximum number of pages will not be penalized.
Proposers are encouraged to submit concise but descriptive proposals. Specific examples of problems, approaches, or goals are preferred to qualitative generalities. Additional information not explicitly called for in the Technical & Management Proposal must not be submitted, but may be included as links in the bibliography. Any additional materials included in the proposal package will be considered for the reviewers’ convenience only and not evaluated as part of the proposal.
The Technical & Management Proposal must not exceed eight (8) pages.

1. Proposal Summary (Recommended 2 pages)
A. Discussion Provide a synopsis of the proposed project, including answers to the following questions:
· What is the intended work the proposer attempts to accomplish?
· How is the proposed work leading to improved health outcomes or improvements in the reliability of health systems?
· How is it done today, and what are the limitations of present approaches?
· What is new in your approach? Why do you think it will be successful at this time?
· What are the key technical, programmatic, or translational risks in your approach, and how do you plan to mitigate or overcome these?
· Who or what will be affected, what will be the impact if the work is successful?
· What are the specific applications of your project?
· To ensure equitable access to all people, how will cost, accessibility, and user experience be addressed?
· How might this effort be misperceived or misused (and how can you prevent that from happening)?
The summary should include a description of the key technical challenges, a concise review of the technologies proposed to overcome these challenges and achieve the project’s goal, and a clear statement of the novelty and uniqueness of the proposed work.
Describe how, if successful, the proposed effort will make a difference (both qualitatively and quantitatively). Describe the innovative aspects of the project in the context of existing capabilities and approaches. Clearly delineate the relationship of this work to other related projects from the past and present. Address plans and risks to achieve accessibility and adoption of technology. This section should include a summary of specific outcomes from the proposed research.
2. Technical Plan (Recommended 5 pages)
2.1 Present a credible plan to achieve the project goal. High-risk, high-reward projects are encouraged.
2.2 Outline and address the technical challenges inherent in the approach and possible solutions for overcoming potential problems.
3. Capabilities/Management Plan (Recommended 1 page)
3.1 Provide a summary of the team’s expertise, including any sub-awardees/consultants and key personnel who will execute the work.
3.2 Identify a principal investigator (PI) for the project and provide a clear description of the team’s organization, including roles and responsibilities.
4. Bibliography (Optional, does not count towards page limit) Provide a brief bibliography with each key personnel’s qualifications (resume), links to relevant papers, references, reports, etc.
