About this file
This is a Virginia State Corporation Commission (SCC) registration form attachment required for bidders responding to IFB 57-26-SKL for Sodium Hexametaphosphate (Glassy Plate Form) in Virginia. The form establishes SCC compliance requirements that bidders must satisfy by demonstrating one of four qualifying conditions: registration as a Virginia corporation or business entity with a valid SCC identification number, operation as a non-corporate business entity, qualification as an out-of-state business entity with no regular and continuous business presence in Virginia, or submission of legal counsel opinion documenting out-of-state status and explaining why Virginia contacts do not constitute transacting business under Virginia Code sections 13.1-757 or related provisions in Titles 13.1 or 50.
Bidders unable to complete any of the foregoing options but with pending SCC applications may request a waiver to submit their SCC identification number after the bid due date, subject to the Commonwealth's sole discretion in determining waiver eligibility. This form is a mandatory compliance document that must be completed and submitted with bids to establish the bidder's legal status and registration requirements relative to Virginia business operations.
