About this file
This is a Past Performance Questionnaire template required for offerors responding to a federal contract opportunity. Offerors must document between three and five (3-5) contracts completed within the last three years that are similar in size and scope to the contract being evaluated. Contracts involving the National Cemetery Administration (NCA) must be listed first, followed by non-NCA contracts. The form requests contractor information including company name, address, telephone number, and email address.
For each contract reference, offerors must provide comprehensive contract details including the client company/agency name, contract number, contract type, dollar value, award date, and completion status. Additionally, offerors must identify whether they served as prime contractor or subcontractor and supply complete point of contact information for the client organization, including the contract person's name, title, address, telephone number, and email. The questionnaire requires a detailed description of supplies and services provided, work location, and relevancy to the current opportunity, along with an assessment of product/service complexity. Offerors must also specify the percentage of work completed by their company versus subcontractors, with explanation of tasks performed if the company completed less than 100% of the work. This template may be duplicated as needed to submit the required number of contract references.
