About this file
This is a Vendor Questionnaire appendix that vendors must complete as part of their proposal submission for solicitation 36C25726R0014, the Dallas VA Medical Center's non-emergency wheelchair transportation services contract. The questionnaire requires vendors to address three primary technical factors: a phase-in transition plan describing the 30-day period following award to prepare for service initiation; a proposed fleet breakdown including staffing plan and dispatch procedures in accordance with Performance Work Statement (PWS) paragraphs 4.4 and 4.5; and a quality control plan per PWS paragraph 6.1. Additionally, vendors must provide attestation via signature that personnel will comply with contractor personnel requirements outlined in PWS paragraph 4.3.
Vendors must indicate their ability to meet all areas and specifications within the PWS and include page number references for each response within their proposal package. The questionnaire also requires completion of FAR Clause VAAR 852.219-75, the VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction. This template serves as a structured checklist to ensure vendors comprehensively address all mandatory technical and compliance requirements before award.
