About this file
This is an Annual Affidavit document for Request for Bid (RFB) 758 2600000690 issued by the Kentucky State Parks system for Old Hickory Furniture procurement. The affidavit serves as a required annual certification submission by vendors interested in supplying furniture products to Kentucky State Parks. The document establishes the framework for ongoing vendor compliance and qualification maintenance throughout the contract period. Specific details regarding product specifications, quantities, delivery schedules, and bidding timelines would be outlined in the primary RFB documentation, with this affidavit functioning as a supporting compliance instrument to verify vendor eligibility and business standing on an annual basis.
The affidavit requires vendors to certify various business conditions and compliance requirements as a condition of contract participation and continuation. Key certifications typically address vendor status, regulatory compliance, and adherence to state procurement requirements. This document functions as an administrative mechanism to ensure that selected vendors maintain their qualifications and meet Kentucky's contracting standards throughout the contract term. Vendors must submit this affidavit annually to remain eligible for order placement under the awarded contract, with failure to submit resulting in potential suspension or termination of procurement authorization.
